You are able to enter all information needed to file an insurance claim in COUNSOL. Please assure your name, your mailing address, and birthdate are exactly the same as they are entered on your insurance card. Failure to have any one of these things entered incorrectly will result in delays in your claim. Provide a copy of your insurance card by taking a picture and downloading into your Counsol record. I will call and confirm coverage before your first appointment.
As noted in policy and procedures you are required to input a
credit card for co-pays before your first visit. Your credit card
information is encrypted and safe on this site. It is
charged when you have an unpaid balance due to a co-pay amount.
